Vascular Ehlers-Danlos Syndrome (vEDS)

Vascular Ehlers-Danlos syndrome (vEDS) is the most dangerous subtype of Ehlers-Danlos syndrome. It is an autosomal dominant disorder marked by widespread fragility of tissues whose extracellular matrix is rich in type III collagen. It is caused by heterozygous mutations in COL3A1, which disrupt the synthesis, secretion or structure of the pro-alpha1(III) collagen chain. With far less functional type III collagen, blood vessel walls, the smooth muscle of the colon and the uterine muscle lose their strength, creating a high risk of aneurysm, dissection or spontaneous vessel rupture that directly threatens the patient's life.

At Haute-Derma, for high-risk connective tissue disorders such as vEDS, our management goes well beyond the skin. We focus on early phenotype recognition and multidisciplinary diagnosis (cardiology, molecular genetics, vascular surgery) to make prevention of acute complications as effective as possible.

A Specialist Medical Approach to Vascular Ehlers-Danlos Syndrome (vEDS)

Because the tissues are extremely fragile and tear easily (tissue friability), our clinical approach at Haute-Derma follows strict professional rules:

  • Hemodynamic risk management: we coordinate medical treatment to keep systolic blood pressure and heart rate optimally low, reducing shear stress on the walls of large arteries and on the skin's microcirculation.
  • No invasive or high-pressure interventions: we rule out cosmetic dissection techniques, non-urgent invasive surgery, strong mechanical pressure treatments (such as cupping or deep massage) and high-heat energy devices that damage the tissue matrix.
  • Mechanical protection and support of the extracellular matrix (ECM): we favor supportive therapies that protect small vessels and supply the co-factors needed to stabilize cross-linking of the remaining collagen.

Specialist Screening and Diagnosis of vEDS at Haute-Derma

To detect vEDS early and stratify risk accurately before a serious vascular event, Haute-Derma uses a standardized workup:

  • Phenotypic evaluation: we look for the hallmark signs — thin, translucent skin with visible veins over the chest and abdomen, premature aging of the skin of the hands and feet (acrogeria), thin atrophic scars, and characteristic facial features (prominent eyes, thin nose, thin lips, hollow cheeks).
  • Family tree and history of vascular events: a detailed family history, focusing on unexplained sudden deaths at a young age, arterial rupture, sigmoid colon perforation or obstetric complications (uterine rupture in pregnancy).
  • Non-invasive vascular imaging: we arrange whole-body magnetic resonance angiography (MRA) or CT angiography (CTA), from the skull base to the pelvis, for regular screening of silent aneurysms or dissections.
  • Molecular genetic confirmation: next-generation sequencing (NGS) or Sanger sequencing covers the entire coding region and intron–exon boundaries of COL3A1 to identify pathogenic variants.

Clinical Management and Specialist Skin Care

Medical care for vEDS requires close multidisciplinary teamwork and very safe, conservative measures:

  • Medical therapy to protect vessel walls: we monitor treatment with celiprolol (a beta-1 blocker with partial beta-2 agonist activity), which has been shown in clinical studies to markedly reduce the rate of arterial rupture in vEDS. High-dose vitamin C (1–2 g/day) is added to support hydroxylation of proline and lysine during collagen synthesis.
  • Skin care that spares the microcirculation: barrier-repair products with ceramides, low-concentration hyaluronic acid and topical vitamin K oxide help reinforce the epidermis and reduce purpura and spontaneous bruising (ecchymoses).
  • Hematoma management: bleeding under the skin is treated with gentle compression using soft protective gauze. Strongly adhesive tape must never be used, because removing it can tear the skin. We monitor closely to prevent compartment syndrome or secondary tissue necrosis.
