Pili Annulati – PA

Pili annulati (PA), also called ringed hair, is a rare inherited abnormality of the hair shaft. Viewed in reflected light, each hair shows alternating light and dark bands along its length, giving the hair a distinctive sparkling or "ringed" look.

It is usually passed on in an autosomal dominant pattern with variable expressivity, although sporadic cases have also been described. The responsible locus has been mapped to chromosome 12q13, a region that holds a cluster of genes for hair cortex keratins.

How pili annulati works at the molecular and optical level:

  • Air Pockets in the Hair Cortex (Cortical Air Vacuoles): During keratinization in the hair follicle matrix, the structure of the cortex is disrupted, leaving small abnormal air-filled cavities between the keratin fiber bundles.
  • An Optical Paradox (Reflected vs. Transmitted Light):
    • In reflected light (clinical exam and trichoscopy): The air pockets act like tiny prisms, reflecting incoming light almost completely, so the affected segments look bright or silvery white.
    • In transmitted light (light microscopy): The same air pockets block and scatter light, so those very segments appear as dark or black bands alternating with normal, lighter cortex.
  • Hair Strength: Unlike most other hair shaft disorders (such as monilethrix or pili torti), the air pockets in pili annulati usually do little to weaken the keratin. Most patients' hair grows to a normal length without widespread breakage, although some more severe cases show mildly increased fragility (weathering).

Pili annulati is a harmless cosmetic variation. What matters most at Haute-Derma is telling it apart from pseudo-pili annulati, an optical effect caused by light reflecting off twisted, flattened hair shafts with no air pockets inside, and from monilethrix (beaded hair), which carries a much higher risk of breakage.

Clinical Features and Trichoscopy / Microscopy Findings

Signs of pili annulati usually show up in early childhood or once the hair grows longer:

  • Clinical Signs:
    • The hair looks unusually shiny, "banded" or iridescent in sunlight or dim artificial light.
    • Up close, each strand shows a regular pattern of light and dark segments along the shaft.
    • Hair length is normal, the scalp is fully covered, and there are no bald patches or scalp inflammation.
    • The banding may be limited to scalp hair or may also involve underarm, pubic and eyebrow hair.
  • Trichoscopy and Microscopy:
    • Trichoscopy: Evenly spaced bright white bands run along the hair shafts, on an otherwise completely healthy scalp.
    • Light Microscopy (Transmitted Light): The bands appear "reversed": segments that look white on clinical exam show up as dark, opaque areas filled with air bubbles, alternating with normal translucent cortex.

Little or no breakage.Very brittle; breaks close to the scalp, with follicular papules.Breaks very easily; hair is short, coarse and dry.

Management and Hair Care at Haute-Derma

Pili annulati is benign, poses no health risk and does not lead to baldness. Care focuses mainly on reassurance and on protecting the hair from chemical and physical damage:

Reassurance and Cosmetic Advice

  • Reassurance: Patients and families should understand that this is a harmless inherited trait of hair keratin. It does not cause major hair loss and does not affect physical development.
  • Embracing the Look: Many people with pili annulati find their hair's natural metallic shimmer in the light uniquely attractive.

Minimizing Heat and Chemical Damage

  • Although hair with pili annulati is fairly strong, the air pockets in the cortex can make it more prone to wear from the environment (weathering).
  • We recommend:
    • Limiting bleaching, coloring, perming and high-heat styling, since heat can expand the air pockets and cause split ends.
    • Using conditioners and hair treatments with silicones or hydrolyzed keratin to fill surface gaps, smooth the hair and reduce friction.
    • Wearing a hat in strong sunlight to protect the keratin from oxidative damage.
