Insect Bites

Insect bites are among the most common skin complaints and are caused by blood-feeding members of the class Insecta, such as mosquitoes, blood-sucking bugs, fleas, bed bugs and lice. Unlike defensive stings, which inject venom that can be necrotizing or neurotoxic, a feeding bite mainly introduces saliva containing enzymes, foreign proteins and anticoagulants into the skin. Reactions range from acute itchy red wheals and persistent papular urticaria to blisters, and can be complicated by staphylococcal or streptococcal infection or by post-inflammatory hyperpigmentation.

At Haute-Derma, we manage insect bite reactions by quickly damping down the hypersensitivity response, controlling flares of itch, breaking the scratch cycle that damages the skin barrier and supporting optimal tissue repair.

Pathophysiology

How Insects Pierce the Skin and Feed

Insects use specialized mouthparts (the proboscis) to pierce the epidermis and reach the small blood vessels of the upper dermis. While feeding, they continuously pump saliva into the surrounding tissue, which serves several purposes:

  • Anticoagulants: apyrase and inhibitors of factor Xa and thrombin stop the blood from clotting locally so it keeps flowing into the proboscis.
  • Vasodilators: nitric oxide, hyaluronidase and bradykinin-like peptides widen the small vessels and increase blood flow to the bite site.
  • Transient immune suppressants: small proteins mask the initial pain so the host does not notice the bite.

Immune and Cellular Responses in the Skin

  • Type I (immediate) hypersensitivity: specific IgE antibodies on mast cells recognize proteins in the insect’s saliva and trigger the release of histamine, serotonin and leukotrienes. Within minutes, this causes vasodilation, leaky vessels and a red wheal with intense itching.
  • Type IV (delayed) hypersensitivity: after 2–6 hours, a T-cell–mediated response draws neutrophils, eosinophils and macrophages into the dermis. The lesion becomes a firm, indurated papule that can persist for days to several weeks.

Clinical Features of Common Insect Bites

Bed Bugs (Cimex lectularius)

Typical lesions are clusters of raised, very itchy red papules measuring 3–10 mm. Bites often line up in a row or a run of 3–4 on exposed skin that touches the mattress, such as the neck, shoulders, arms and back. A tiny central hemorrhagic dot is common.

Fleas (Siphonaptera)

Small raised papules, 1–3 mm across, surrounded by a spreading red halo. Fleas tend to bite the lower legs, ankles and areas where clothing fits tightly. The itch is intense and nagging, and scratching leads to crusted, excoriated skin.

Mosquitoes and Blood-Sucking Bugs (Triatominae)

The first lesion is a well-defined red wheal with a central puncture point. In strongly allergic people, it may progress to a tense, clear fluid-filled blister (bullous bite reaction) or to persistent, itchy urticarial papules.

Assessment and Treatment at Haute-Derma

  • Cleansing and reducing local swelling: The affected skin is cleaned with a mild antiseptic such as 0.2% chlorhexidine or 0.9% saline. A cold compress for 10–15 minutes then reduces vasodilation and swelling and brings immediate itch relief.
  • Controlling inflammation and allergy: A medium- or high-potency topical corticosteroid such as hydrocortisone butyrate 0.1% or betamethasone dipropionate 0.05% is applied twice daily for 5–7 days. An oral second-generation H1 antihistamine, such as levocetirizine 5 mg daily or fexofenadine 180 mg daily, is added to control the wider reaction.
  • Preventing infection and restoring the skin barrier: If there are scratch marks or signs of infection, we prescribe an antibiotic cream with mupirocin 2% or fusidic acid 2%. Once the acute inflammation settles, a repair cream with panthenol, madecassoside and niacinamide helps the epidermis regenerate and prevents post-inflammatory hyperpigmentation.
