Acanthosis Nigricans Maligna

Malignant acanthosis nigricans (MAN) is a rare but important paraneoplastic skin syndrome, marked by rapidly spreading hyperkeratosis and hyperpigmentation over large areas. Unlike the benign forms of acanthosis nigricans linked to obesity or insulin resistance, MAN points directly to an underlying adenocarcinoma. Gastric adenocarcinoma accounts for 60–70% of cases, followed by cancers of the colon and rectum, pancreas, liver, lung, and ovary.

At the molecular level, MAN develops because the internal tumor releases excessive amounts of circulating factors, particularly transforming growth factor alpha (TGF-alpha), epidermal growth factor (EGF), and IGF-1. High blood levels of TGF-alpha bind to and continuously switch on EGFR in the basal layer of the epidermis. This activates the MAPK/ERK signaling pathway, driving a surge in cell division, thickening of the stratum corneum, and abnormal melanocyte distribution.

At Haute-Derma, we consider recognizing MAN with digital dermoscopy a key screening opportunity. Spotting acanthosis nigricans that is progressing in an unusual way lets the dermatologist send the patient for gastrointestinal cancer screening within a critical window, before the internal tumor spreads to distant sites.

Molecular Mechanisms and Clinical Features of MAN

Malignant acanthosis nigricans looks clearly different from the common metabolic form:

  • Sudden onset and rapid spread: It develops quickly, over weeks to a few months. Thickened gray-brown to jet-black plaques with a rough, velvety surface spread extensively through the skin folds (neck, armpits, groin, and under the breasts).
  • Involvement of unusual sites and mucous membranes:
    • Mucosal lesions (more than 50% of cases): Unlike the benign form, MAN extends onto the lips, tongue, gums, and palate as rough, velvety papillomatous plaques (usually not pigmented, but they can feel bulky and make swallowing painful).
    • Palm and sole involvement (tripe palms / pachydermatoglyphy): The palms become thick and velvety, with deep, exaggerated skin ridges that look like the lining of a cow's stomach (tripe).
  • Associated paraneoplastic triad: It often appears together with the sign of Leser-Trélat (a sudden eruption of many itchy seborrheic keratoses) and with marked weight loss, poor appetite, and wasting caused by the internal tumor.

Cancer Screening and Clinical Management at Haute-Derma

When we see a patient with suspected malignant acanthosis nigricans, Haute-Derma follows a strict diagnostic and safety process:

Dermoscopy and Skin Ultrasound

  • Digital dermoscopy: We look for finger-like epidermal projections, characteristic deep pigmented furrows, and widespread hyperkeratosis to rule out common keratinizing skin conditions.
  • High-frequency skin ultrasound (HFUS): Measures the thickness of the stratum corneum, mild thickening of the spinous layer, and increased superficial dermal blood vessels.

Invasive Cosmetic Procedures Are Contraindicated

  • No ablative laser or peels: CO2 or erbium:YAG lasers, strong chemical peels, and microneedling must never be used to "lighten" or "resurface" the plaques. Invasive treatment on skin that is being driven by circulating TGF-alpha leads to persistent ulceration and necrosis, infection, and a more severe flare.
  • Gentle local care: We prescribe only a barrier-repair moisturizer with a 3:1:1 lipid ratio, a soothing hypochlorous acid (HOCl) solution, and a low-strength topical retinoid under close medical supervision.

Urgent Oncology and Gastroenterology Referral

  • We prepare a detailed medical record and refer the patient urgently to a leading oncology and gastroenterology center for:
    • Upper endoscopy and colonoscopy: To look for gastric adenocarcinoma or malignant colon polyps.
    • Whole-body CT or PET-CT: To screen for tumors of the pancreas, liver, lung, and ovary.
    • Tumor marker tests: CEA, CA 19-9, and CA 125 levels.
