CARD14-Mediated Pustular Psoriasis
CARD14-mediated pustular psoriasis (also known as PSORS2) is a rare inherited form of psoriasis caused by mutations in the CARD14 gene. This gene acts like a "switch" that turns on inflammatory pathways inside epidermal cells, so when it is faulty the result is chronic skin inflammation punctuated by severe flares of pustules.
At Haute-Derma, we treat this condition not as an ordinary skin problem but as a systemic autoinflammatory disorder. Using genetic testing and targeted biologic therapy, we help patients get control of the underlying "inflammation switch," restoring healthy skin and doing everything possible to prevent future flares.
Our Approach to CARD14-Mediated Pustular Psoriasis
Our specialists at Haute-Derma know that patients carrying a CARD14 mutation often respond differently from those with common psoriasis. That is why our process always starts with genetic analysis to pin down the exact disease subtype.
We are early adopters of the newest generation of biologics, which act directly on the cytokines downstream of CARD14, such as IL-17 and IL-23. At Haute-Derma, the goal is a lasting immune balance, so you no longer have to rely on short-term topical treatments or oral corticosteroids with their many side effects.
Our in-depth management program includes:
- Molecular genetic screening to identify CARD14 mutations.
- Biologic therapy tailored to the level of gene activity.
- An anti-inflammatory lifestyle plan to keep the disease in long-term remission.
How We Diagnose CARD14-Mediated Pustular Psoriasis at Haute-Derma
Because the disease can look very different from one patient to the next, Haute-Derma uses a multi-step diagnostic process to make sure the diagnosis is accurate:
- Clinical phenotype assessment: We look for thick red psoriatic plaques, often with sudden crops of sterile pustules and sometimes a picture of generalized erythrodermic psoriasis.
- Family history: We take a careful family history, since this is an autosomal dominant condition (PSORS2).
- Dermoscopy: We examine the characteristic dotted vessels and the pattern of scaling to tell it apart from other forms of dermatitis.
- CARD14 gene sequencing: This is the deciding test for a definitive diagnosis, clearly separating it from ordinary plaque psoriasis and from DITRA (caused by IL36RN mutations).
- Specialized skin biopsy: We analyze the white blood cell infiltrate and signs of epidermal overgrowth driven by excessive CARD14 activation.
How Is CARD14-Mediated Pustular Psoriasis Treated at Haute-Derma?
Our treatment plan aims to "switch off" the misfiring inflammatory signals at their source:
- IL-23 and IL-17 inhibitor biologics: Because CARD14 strongly drives the IL-23/IL-17 axis, biologics aimed at these targets clear the skin exceptionally well and keep results durable.
- Systemic retinoids (acitretin): These help normalize keratinization and epidermal cell differentiation, thinning out thick, scaly plaques.
- Managing joint complications: We screen early for associated psoriatic arthritis so we can step in promptly and protect mobility.
- Adjunctive NB-UVB phototherapy: Narrowband ultraviolet light suppresses local cell overgrowth and is a strong support during the maintenance phase.
- Controlling environmental triggers: We coach patients on avoiding triggers such as viral or bacterial infections, skin injury (the Koebner phenomenon) and psychological stress.
