Skin Manifestations of Drug Abuse

Skin damage from substance use disorders (SUD) results from a combination of direct chemical toxicity, changes in the endothelium and microcirculation, allergic or contaminant reactions, and direct mechanical injury.

Addictive substances (heroin, cocaine, methamphetamine, ketamine, alcohol) harm the skin through four main mechanisms:

Microvascular Constriction and Ischemic Necrosis

  • Stimulants such as cocaine and methamphetamine trigger a powerful release of catecholamines (norepinephrine, dopamine), causing prolonged spasm of the small arteries under the skin.
  • The sudden loss of blood supply kills epidermal keratinocytes, disrupts the basement membrane and produces sharply demarcated, dirty-looking necrotic ulcers (ischemic ulcers).

Foreign-Body Granulomas and Embolism From Contaminants

  • People who inject drugs (PWID) often dissolve tablets containing talc, microcrystalline cellulose or starch, or inject drugs cut with toxic adulterants such as levamisole (found in cocaine).
  • These microscopic particles can block small vessels (microvascular embolization) or provoke a foreign-body granulomatous reaction, leading to firm nodules under the skin, necrotizing small-vessel inflammation (leukocytoclastic vasculitis) and immune-complex deposition.

Local and Systemic Immune Suppression

  • Morphine and other opioids bind to mu-opioid receptors on immune cells (macrophages, T cells, NK cells), reducing their ability to engulf microbes and release protective cytokines.
  • Skin bacteria, especially methicillin-resistant Staphylococcus aureus (MRSA), easily enter through non-sterile injection sites and can cause life-threatening deep necrotizing infections.

At Haute-Derma, recognizing the skin signs of substance use early helps our doctors uncover hidden conditions (HIV, hepatitis B/C, infective endocarditis) and stop tissue destruction before it leaves permanent, disfiguring scars.

Clinical Signs by Route of Use

Signs of Injection Drug Use

  • Track marks: Bands of fibrotic, hyperpigmented scarring along superficial veins, usually in the elbow crease, forearms, backs of the hands and tops of the feet. They result from repeated injections that cause chemical phlebitis and extensive fibrosis.
  • Skin popping (subcutaneous injection) burns and ulcers:
    • Once the superficial veins are completely scarred, users switch to injecting directly into the subcutaneous tissue or muscle.
    • This produces blisters, deep abscesses or round, "punched-out" ulcers, which heal as very characteristic round, depressed atrophic scars.
  • Puffy hand/foot syndrome: Firm, non-pitting swelling on the backs of the hands or feet, caused by repeated injections that severely damage the superficial blood vessels and permanently obstruct lymphatic flow.

Signs of Snorting, Smoking and Systemic Toxicity

  • Damage from cocaine and levamisole contamination:
    • Nasal septal perforation: Snorted cocaine causes intense constriction of the vessels in the nasal lining, leading to necrosis of the septal cartilage.
    • Levamisole-induced vasculitis: Purple-black hemorrhagic, necrotic patches (retiform purpura) appear typically on the ears, nose and limbs, along with a low neutrophil count (agranulocytosis).
  • "Cocaine bugs" / "meth mites" (the sensation of insects crawling on the skin):
    • Much like delusional parasitosis, the dopamine surge caused by methamphetamine or cocaine creates a false sensation of bugs burrowing under the skin. Patients dig at their skin with fingernails, knives or needles, leaving ragged, excoriated ulcers (meth sores), often alongside severe tooth decay (meth mouth).

Diagnosis, Acute Care and Skin Recovery at Haute-Derma

At Haute-Derma, skin damage in patients with past or current substance use is managed using our Multi-Tier Skin Repair Protocol:

In-Depth Screening and Whole-Body Assessment

  • Essential laboratory tests: Wound cultures (for MRSA and anaerobic bacteria), screening for bloodborne infections (HIV, hepatitis B/C, syphilis), ANCA testing and a complete blood count (to check neutrophils if levamisole vasculitis is suspected).
  • Circulation assessment: Doppler ultrasound of the superficial vessels to rule out deep vein thrombosis (DVT) or arterial blockage in the legs.

Acute Wound Management and Cleaning

  • Necrotizing infection: Surgical debridement removes all black necrotic tissue and foul pus. The wound is irrigated with 10% povidone-iodine or 0.05% chlorhexidine.
  • Antibiotic therapy: During the acute phase, broad-spectrum antibiotics covering MRSA and anaerobes are combined (for example, IV vancomycin or linezolid together with metronidazole).
  • Wound dressings: Silver-containing dressings (silver Hydrofiber dressings) or hydrocolloid dressings keep the wound in a moist, physiological healing environment.

Scar Repair and Skin Regeneration at Haute-Derma (Stable Phase)

  • Restoring the skin barrier and microbiome: Repair creams with skin-identical lipids (ceramide NP/AP/EOP, cholesterol, fatty acids), combined with ectoin and madecassoside, reduce inflammation and speed re-epithelialization.
  • Low-level light therapy (LLLT, 630 nm + 830 nm): Red and near-infrared light stimulates cellular mitochondria to produce more ATP, speeding wound healing and helping rebuild damaged capillary networks.
  • Advanced treatment for fibrotic scars and hyperpigmentation:
    • Fractional CO2 laser (10,600 nm): Creates tiny ablative thermal zones (micro-ablative zones) that break up the hard, distorted scar bands left by old injection sites (track marks) and stimulate new, well-organized collagen.
    • Picosecond laser (532 nm/1064 nm): Breaks up patchy dark post-inflammatory hyperpigmentation (PIH) and clears contaminant deposits that leave dark discoloration under the skin.
