Steatocystoma
Steatocystoma — which occurs either as a solitary lesion (steatocystoma simplex) or as many lesions (steatocystoma multiplex) — is a benign cyst arising from the sebaceous duct of the pilosebaceous unit. Unlike an epidermoid cyst, which is packed with flaky keratin, or a pilar cyst, which contains dense, compact keratin, a steatocystoma holds an oily, buttery or serous fluid secreted straight into the cavity by mature sebaceous lobules in its wall.
KRT17 Mutations and Inheritance
- Inheritance: Steatocystoma multiplex is mostly inherited in an autosomal dominant pattern with high penetrance.
- Keratin 17 (KRT17) mutations: Molecular genetic studies have identified point mutations in the KRT17 gene (on chromosome 17q21.2) that alter the structure of intermediate filaments in the epithelium of the follicular infundibulum and sebaceous duct. The result is abnormal keratinization, which plugs the sebaceous duct and causes it to balloon into a cyst.
- Clinical association: KRT17 mutations also cause pachyonychia congenita type 2 (Jackson–Lawler syndrome).
Cyst Contents and the Risk of Inflammation
The cyst contains a complex mixture of triglycerides, fatty esters, squalene and flakes of keratin. At first this material is a clear yellow liquid or a thick, butter-like paste. If the cyst is knocked, compressed or squeezed the wrong way, its thin wall can tear open and release lipid and keratin into the surrounding dermis. This sets off a foreign-body granulomatous reaction that turns a harmless cyst into a sterile but very painful pseudo-abscess (suppurative steatocystoma), which often heals with a hypertrophic or depressed scar.
At Haute-Derma, we design treatment for steatocystoma around its histology: a very thin yet tough wall, lined by a distinctive keratinous cuticle and filled with liquid to pasty sebum. Our doctors combine high-frequency skin ultrasound (20–50 MHz) with dermoscopy to map the borders of each cyst and tell whether it is quiet or has already become inflamed and suppurative.
For a single cyst or just a few, Haute-Derma uses a micro-punch technique to open the cyst, drain it and peel out the cyst wall (micro-punch and membrane extirpation). For steatocystoma multiplex with dozens of cystic papules on the chest or back, we use a 2940 nm Er:YAG laser in micro-spot pulses to vaporize a tiny opening in the overlying epidermis, drain the sebum and remove the cyst lining without leaving extensive surgical scars.
Histopathology, Dermoscopy and Ultrasound Criteria for Diagnosis
Characteristic Histopathology
Under the microscope, steatocystoma shows hallmark "gold standard" features that confirm the diagnosis:
- Cyst wall: Thin, made up of 2–5 layers of squamous epithelial cells.
- Wavy eosinophilic cuticle: The inner surface of the wall is covered by a thin keratin layer with no keratohyalin granules. It stains deep pink with eosin and has a very characteristic wavy, crenulated outline.
- Sebaceous lobules in the cyst wall: Mature or flattened sebaceous lobules lie pressed tightly against the epithelial lining, just inside or just outside the cyst wall.
- Cyst lumen: Usually looks empty on H&E sections (the lipid is washed out during tissue processing) or contains amorphous material and a few vellus hairs.
Dermoscopy and High-Frequency Ultrasound Criteria
- Dermoscopy:
- Homogeneous yellowish or creamy structure: The cloudy yellow glow of the oily contents shows up clearly under polarized light.
- Smooth surface with no central punctum: There is no opening onto the skin surface.
- Perilesional linear vessels: Small capillaries ring the base of the papule where the cyst pushes them aside.
- High-frequency ultrasound (20–50 MHz):
- Homogeneous anechoic to hypoechoic cyst: Oval or lobulated, sitting in the deep dermis or upper subcutis.
- No internal echoes or calcification: This immediately sets it apart from a pilar cyst.
- Mild posterior acoustic enhancement: Typical of a structure filled with liquid or semi-liquid contents.
Clinical Features
- Typical sites: Areas rich in sebaceous glands, such as the presternal chest, armpits, neck, arms, back and groin/scrotum.
- Lesions: Smooth papules or nodules 3–20 mm across, lying in or just beneath the skin. They feel soft or firm and elastic. When pricked with a fine needle or pressed gently, they release a cloudy yellow oily or buttery fluid that may smell faintly unpleasant.
- Age of onset: Lesions usually become obvious at puberty or in early adulthood, when the surge in androgens stimulates the sebaceous glands.
Clinical and Histologic Differential Diagnosis
| Steatocystoma | Epidermoid Cyst | Milium |
|---|---|---|
| Liquid to buttery sebum + vellus hairs | Flaky, foul-smelling laminated keratin | Firm, round keratin pearl |
| Distinctive wavy keratinous cuticle | Normal granular layer, no cuticle | Thin granular layer, no cuticle |
| Sebaceous lobules attached to the cyst wall | No sebaceous lobules | No sebaceous lobules |
| No punctum | Punctum often present | No punctum |
| KRT17 gene (multiplex form) | Rarely inherited | Rarely inherited |
Haute-Derma Protocol: Micro-Extraction or Er:YAG Laser Drainage With Tissue Repair
At Haute-Derma, our approach to steatocystoma aims to remove the cyst lining completely while getting the best cosmetic result and avoiding hypertrophic scars on the chest and neck:
High-Frequency Ultrasound Assessment and Lesion Classification
- Mapping borders and inflammation: We check whether the cyst is quiet (containing only sebum) or has ruptured and inflamed the tissue around it.
- Marking skin tension lines (Langer's lines): Each opening is placed along the natural skin creases to keep scarring to a minimum once it heals.
Mini-Punch Microsurgery or Er:YAG Laser for Drainage and Cyst Wall Removal
- Micro-fenestration:
- Large or solitary cysts: A 1.5–2 mm mini-punch makes a tiny opening at the dome of the cyst.
- Steatocystoma multiplex (many cysts): A 2940 nm Er:YAG laser vaporizes a micro-opening in the roof of each cyst.
- Evacuation and membrane extirpation:
- Gentle pressure expresses all of the buttery sebum.
- Curved micro-mosquito forceps are passed through the opening to grip the thin, tough cyst membrane, which is gently twisted and drawn out intact.
Cavity Repair and Prevention of Hypertrophic Scars
- Flushing the cavity: The empty cyst cavity is irrigated with diluted povidone-iodine plus an antibiotic to clear bacteria.
- Regeneration with Pincer Biomodulation: This regenerative molecular technology helps the cavity close quickly and prevents fluid from collecting again.
- 595 nm Vbeam laser / red LED light: Calms redness, encourages healthy collagen formation and lowers the risk of hypertrophic scarring (especially over the breastbone).
