Warty Dyskeratoma

Warty dyskeratoma (WD) is a rare, benign epidermal lesion characterized by abnormal keratinization and focal acantholysis within a cup-shaped depression or hair follicle infundibulum. Although it looks very much like Darier disease (an autosomal dominant genetic disorder) under the microscope, warty dyskeratoma is a solitary lesion that is not inherited and is unrelated to ATP2A2 gene mutations.

Acantholysis and the Formation of Epidermal Clefts

  • Loss of desmosomal connections: WD arises from faulty formation or breakdown of the desmosomes that link keratinocytes just above the basal layer (suprabasal acantholysis).
  • Formation of lacunae: this separation creates spaces within the epidermis (suprabasal clefts or lacunae) filled with detached keratinocytes. Although the overlying spinous cells come apart, the basal cells stay firmly attached to the basement membrane because their hemidesmosomes remain intact.
  • Elongated dermal papillae (villi): the underlying dermal papillae push upward into the lacunae. These projections, known as villi, are covered by only a single row of basal cells, giving a highly distinctive histologic picture.

Abnormal Keratinization (Dyskeratosis)

Within the lesion, keratinization is disordered and happens too early (premature keratinization), producing two characteristic cell types:

  • Corps ronds (round dyskeratotic cells): found in the upper spinous or granular layer. These are enlarged keratinocytes with a shrunken, dark (pyknotic) nucleus, surrounded by a clear cytoplasmic halo and a rim of strongly eosinophilic keratin.
  • Grains: found in the stratum corneum or at the edge of the clefts. These are small, flattened cells with elongated, dark nuclei, packed with abnormal keratohyalin granules.

Warty dyskeratoma is a localized, benign lesion of the follicular infundibulum: a deep, cup-shaped pit filled with keratin, not an HPV wart and not an invasive cancer. With this in mind, Haute-Derma has developed a conservative approach to diagnosis and treatment. We combine broad-spectrum dermoscopy to outline the central keratin rim with high-frequency skin ultrasound to measure the depth of the follicular pit. We then use layer-by-layer Er:YAG 2940 nm laser micro-ablation to remove the entire keratin-filled pit down to the basement membrane without harming the surrounding healthy tissue, followed by PDRN biostimulation to speed healing, aiming for smooth skin without pitted scars.

Histopathology and Dermoscopic Diagnostic Criteria

Characteristic Microscopic Findings

Warty dyskeratoma has a very clear and distinctive architecture under the microscope:

  • Cup-shaped invagination: at the center, the epidermis folds deep into the papillary dermis, and this depression is filled with a dense keratin plug.
  • Suprabasal lacunae: keratinocytes separate just above the basal layer, forming clefts that contain floating acantholytic cells.
  • Finger-like dermal papillae (villi): dermal papillae project freely into the lacunae, lined by a single row of flattened or short columnar basal cells.
  • Corps ronds and grains: dyskeratotic cells are abundant in the superficial epidermis around the opening of the pit.

Key Dermoscopic Criteria

Digital dermoscopy at Haute-Derma looks for the following signs to confirm WD:

  • Central yellow-brown keratin plug: a compact mass of keratin in the center of the lesion, which may resemble a giant blackhead or a crater-like wart.
  • Slightly raised whitish-pink halo: the surrounding rim is gently elevated, pale pink or light gray, and clearly demarcated from normal skin.
  • Peripheral radial or dotted vessels: dotted or short linear vessels arranged radially around the base of the keratin plug in a crown-like pattern.

Laser Micro-Ablation and Biological Repair at Haute-Derma

At Haute-Derma, our treatment of warty dyskeratoma aims to remove the base of the keratin-filled pit completely, without a wide surgical excision that could leave an unsightly scar on the face:

High-Frequency Ultrasound and Dermoscopic Assessment

The lesion is scanned with 20–50 MHz skin ultrasound together with dermoscopy. The doctor measures the exact depth of the keratin-filled pit (usually 1.5–3 mm) and assesses the underlying basement membrane to choose the right laser settings.

Micro-Ablation With Er:YAG 2940 nm / SuperPulse CO2 Laser

  • Er:YAG 2940 nm laser (gold standard): the Er:YAG beam vaporizes tissue with micron-level precision. The doctor first clears the central keratin plug, then removes the entire acantholytic epidermis lining the pit layer by layer.
  • Preserving the basement membrane: ablation stops exactly at the basement membrane level identified on the high-frequency ultrasound assessment, eliminating the source of keratin buildup without damaging the healthy dermis.

Biological Repair and Prevention of Pitted Scarring

  • Regenerative boost: stem cell exosomes and salmon DNA PDRN are micro-injected directly into the base of the ablated pit, encouraging healthy keratinocytes from the surrounding edge to migrate in and fill the defect.
  • Moist wound care: a hydrocolloid dressing maintains an ideal moist environment, so the base of the wound fills in level with the surrounding skin within 5–7 days.
