Pleomorphic Lipoma

Pleomorphic lipoma (PL) is an uncommon, distinctive benign variant of lipoma that belongs to the same genetic and histologic spectrum as spindle cell lipoma (SCL). Its hallmark is multinucleated "floret-like" giant cells mixed with mature fat cells, spindle cells, ropey bands of collagen and a myxoid (mucinous) background.

The word "pleomorphic" in the name often alarms patients and can challenge pathologists, because the bizarre multinucleated cells seen under the microscope are easily confused with well-differentiated liposarcoma (WDLPS) or pleomorphic liposarcoma. Biologically, however, pleomorphic lipoma is benign. It occurs mainly in older men (50–70 years), on the back of the neck, shoulders and upper back, and the outlook after complete removal is excellent.

At Haute-Derma, we approach the diagnosis with a standardized pathway: microscopic examination plus an immunohistochemistry and molecular marker panel (CD34/Rb/MDM2), 3.0T MRI imaging, and microsurgical removal of the tumor with its capsule intact — giving patients peace of mind and a good cosmetic result.

Clinical and Pathological Features

Causes and Molecular Genetics

  • Loss of the RB1 tumor suppressor gene (retinoblastoma gene):
    • Like spindle cell lipoma, pleomorphic lipoma carries chromosomal changes at 13q14 or 16q that lead to loss of Rb protein expression (Rb-negative).
    • Loss of Rb on immunohistochemistry reliably separates PL from malignant fatty tumors, which retain Rb expression (Rb-positive).
  • Who Gets It and Where:
    • Strongly favors men aged 50–70 (>85% of cases).
    • Typical sites: the back of the neck (nuchal region), shoulders and upper back. The tumor sits in the subcutaneous fat, grows slowly, is painless and well circumscribed.

Histopathology (the Gold Standard)

Under the microscope, pleomorphic lipoma has a textbook picture made of four intertwined components:

  • Floret-Like Giant Cells (the Signature Finding):
    • The core microscopic criterion for PL. These are large multinucleated cells whose nuclei overlap in a ring around the edge of the cell, like the petals of a daisy.
    • Despite their odd-looking nuclei, these cells show no abnormal mitoses or necrosis.
    • Mature adipocytes: Uniform fat cells, with no malignant lipoblasts.
    • Uniform spindle cells and rope-like collagen: Thick, glassy, refractile collagen bundles lie between the spindle cells.
    • Myxoid stroma: Contains anywhere from scant to abundant mucopolysaccharide.

Differential Diagnosis and Immunohistochemistry (IHC)

Separating pleomorphic lipoma from malignant fatty tumors is critical to avoid unnecessarily extensive, destructive surgery:

ConditionImmunohistochemistry (IHC)Distinguishing Microscopic Features
Pleomorphic lipoma (PL)Strong CD34 (+); loss of Rb (Rb −); S100 (+); MDM2 (−); CDK4 (−)Floret-like giant cells; uniform fat cells; rope-like collagen; no malignant lipoblasts.
Well-differentiated liposarcoma (WDLPS / ALT)Strong MDM2 (+); strong CDK4 (+); MDM2 gene amplification (+); Rb (+)Atypical lipoblasts with hyperchromatic nuclei; fat cells of varying size; no floret-like cells.
Pleomorphic liposarcoma (PLS)MDM2 (+/−); very high Ki-67 (>30–50%); Rb (+)Numerous abnormal mitoses; extensive necrosis; extreme nuclear atypia with obvious malignant change.
Spindle cell lipoma (SCL)CD34 (+); loss of Rb (Rb −); MDM2 (−)Similar architecture to PL but lacking (or with very few) floret-like giant cells.

Our Approach to Treating Pleomorphic Lipoma

Haute-Derma manages pleomorphic lipoma on three principles: precise microscopic diagnosis, microsurgical removal of the whole encapsulated tumor, and preserving the appearance of the neck and shoulders:

  • Confirming It Is Benign With Required Immunostaining: We check three markers: CD34 (+), Rb loss (−) and MDM2 (−). A negative MDM2 helps rule out well-differentiated liposarcoma and takes a great weight off the patient's mind.
  • Microsurgical Complete Encapsulated Excision: Because pleomorphic lipoma has a clear fibrous capsule and lies under the skin, removing the whole tumor microsurgically is usually curative.
  • Cosmetically Planned Incisions on the Neck and Back: Incisions follow the natural skin creases (Langer's lines) of the neck and shoulders to hide the scar as well as possible.

How We Diagnose Pleomorphic Lipoma at Haute-Derma

  • Clinical Exam: We assess a lump under the skin of the back of the neck, shoulders or upper back in a middle-aged or older man. It is soft to slightly firm, moves freely and is not attached to the deep muscle fascia.
  • High-Resolution 3.0T MRI: Shows a well-defined subcutaneous mass with mixed fat signal (high on T1) and fibromyxoid strands. MRI confirms that the tumor is confined to the subcutaneous layer and has not invaded the deep neck muscles.
  • Core Needle Biopsy and IHC: A biopsy sample is stained with H&E (to identify the floret-like giant cells) and run through an immunohistochemistry panel (CD34, Rb, MDM2, CDK4) to confirm the diagnosis.

Specialized Treatment at Haute-Derma

  • Microsurgical Enucleation With the Capsule Intact:
    • Indication: The main treatment for pleomorphic lipoma.
    • Technique: A minimal incision is made along a neck skin crease, and the whole fibrous capsule is carefully dissected away from the surrounding healthy fat without rupturing the tumor. The wound is closed in several layers to eliminate dead space and prevent fluid buildup after surgery.
  • Post-Operative Recovery and Skin Resurfacing: He-Ne laser is used right after surgery to reduce inflammation and swelling, followed by fractional CO₂ laser sessions after suture removal to fade the scar so it becomes smooth and blends with the surrounding skin.
  • Follow-Up: An ultrasound check of the surgical site at 6 months. Because the tumor is benign, no systemic cancer surveillance is needed.
