Chronic Atypical Neutrophilic Dermatosis with Lipodystrophy and Elevated Temperature (CANDLE)
CANDLE syndrome is a rare systemic autoinflammatory disease belonging to the group known as interferonopathies. It is caused by gene mutations (most often in PSMB8) that disrupt the proteasome, the cell's "waste-processing plant." The buildup of faulty proteins drives excessive type I interferon production, resulting in chronic inflammation, high fevers and a characteristic loss of fat beneath the skin.
At Haute-Derma, we treat CANDLE as a medical challenge that calls for early, multidisciplinary care. Our goal is to bring the interferon "storm" under control, halt progressive fat loss and protect each patient's overall physical development, especially in children.
Our Approach to Treating CANDLE
Our specialists focus on blocking the JAK-STAT signaling pathway, the main relay station for interferon inside the cell. We recognize that CANDLE is not only a cosmetic burden because of fat loss; systemic inflammation also causes ongoing pain and fatigue.
We were early adopters of new-generation JAK inhibitors, which can substantially improve the outlook for this disease. At Haute-Derma, every patient is closely monitored with specific biomarkers so the dose can be fine-tuned, with the aim of achieving the best possible disease control.
Our CANDLE care pathway includes:
- Comprehensive gene sequencing (particularly PSMB8) to identify the proteasome mutation.
- Targeted JAK inhibitor therapy to shut down interferon signaling.
- Management of the cosmetic and metabolic complications of lipodystrophy.
How We Diagnose CANDLE at Haute-Derma
At Haute-Derma, diagnosis combines the characteristic clinical picture with molecular testing:
- Recognizing the skin findings: swollen, annular red papules or plaques, usually on the face and limbs.
- Assessing lipodystrophy: tracking the gradual loss of subcutaneous fat, especially in the face (giving a gaunt look), chest and upper arms.
- Checking associated symptoms: recurrent high fevers, periorbital edema and unusually thick lips.
- Interferon signature testing: measuring increased expression of interferon-stimulated genes (ISGs) in the blood to confirm an interferonopathy.
- Skin biopsy: looking for infiltrates of atypical neutrophils and mononuclear cells around blood vessels and sebaceous glands.
How Is CANDLE Treated at Haute-Derma?
Our treatment plan targets the faulty signaling pathway inside the cell:
- JAK inhibitor therapy (baricitinib, tofacitinib): the most important breakthrough. These drugs block inflammatory signals from interferon, bringing fevers down quickly and markedly improving skin lesions.
- Adjunctive low-dose corticosteroids: help control inflammation during the transition period or during severe flares.
- Managing internal organ complications: regular screening for liver and spleen enlargement and for blood abnormalities caused by chronic inflammation.
- Aesthetic restoration: once inflammation is stable, our specialists advise on non-surgical aesthetic options to restore appearance after fat loss.
- Genetic counseling and family screening: helping families understand recessive inheritance and providing a long-term care plan for the patient.
