Proliferating Pilar Tumor

A proliferating pilar tumor (PPT), also called a proliferating trichilemmal tumor, is a rare epithelial growth that arises from the outer root sheath of the hair follicle. It usually appears as a lobulated, cyst-like lump on the scalp that grows slowly over many years but can suddenly enlarge and become necrotic, ulcerated, bleeding, or infected. Most of these tumors are benign, but PPT can undergo malignant change or be seriously confused with squamous cell carcinoma (SCC) if the pathology is not carefully assessed.

At Haute-Derma, we go beyond simply removing the visible lump. We follow a standardized microscopic diagnostic process and a carefully planned treatment approach, so patients can recover their appearance with confidence, lower their risk of recurrence, and keep their scalp healthy.

Our Approach to Treating Proliferating Pilar Tumors

With specialist doctors at the center and international medical standards as our foundation, Haute-Derma manages each case from microscopic diagnosis through to recovery after surgery:

  • In-depth screening and microscopic assessment: Careful histopathologic analysis to clearly separate benign PPT from atypical (borderline) and malignant PPT.
  • Personalized surgical planning: Choosing appropriate surgical margins based on the size and location of the lesion and the degree of cellular atypia, while preserving as much healthy scalp tissue as possible.
  • Microscopic margin control: Modern surgical techniques to achieve clear margins (R0) and remove any remaining tumor cells.
  • Long-term care and follow-up: A schedule of regular post-operative check-ups to catch early signs of local recurrence or abnormal changes in the surrounding follicles.

Diagnosing Proliferating Pilar Tumors at Haute-Derma

To pin down the nature of the tumor and how far it has progressed, Haute-Derma uses a standard multi-step diagnostic process:

  • Clinical examination: Assessing the size, firmness, borders, and mobility of the lump, and any surface necrosis, ulceration, or infection.
  • Medical history: A careful history of the lesion (such as a pre-existing pilar cyst), how quickly it has grown, and any previous injury or irritation.
  • Advanced imaging: High-frequency skin ultrasound, or MRI for large tumors, to see whether the tumor has reached the deep fascia or the periosteum.
  • Biopsy and histopathology: The gold standard. It identifies trichilemmal keratinization (abrupt, compact keratin without a granular layer) and assesses nuclear atypia.
  • Immunohistochemistry (IHC): Markers such as CD34, Ki-67, and p53 when malignant change is suspected, to reliably distinguish it from squamous cell carcinoma (SCC).

How We Treat the Condition at Haute-Derma

Our treatment plan is built on close collaboration between clinical medicine and pathology:

  • Wide local excision (WLE): Complete removal of the tumor with a safety margin of 0.5–1.0 cm (benign tumors) or 1.0–2.0 cm (borderline or malignant tumors) from the visible edge, extending down to the superficial fascia or periosteum.
  • Mohs micrographic surgery (MMS): Preferred for recurrent or large tumors, or for sites where as much healthy scalp as possible must be preserved, because it allows the entire surgical margin to be checked under the microscope.
  • Managing necrosis and infection: Systemic antibiotics plus cleaning and drainage of the lesion before surgery for tumors that have ruptured, become necrotic, or developed a secondary infection.
  • Scalp reconstruction and cosmetic repair: Skin flaps, skin grafts, or other reconstructive techniques to support healing and keep the risk of unsightly scars or wide bald scars as low as possible.
  • Regular follow-up program: Check-ups every 3–6 months to watch for local recurrence and assess the regional lymph nodes.
