Trichilemmoma

Trichilemmoma is a benign adnexal epithelial tumor that differentiates toward the outer root sheath (ORS) of the anagen hair follicle. It is defined by an accumulation of glycogen-rich clear cells surrounded by a thickened hyaline basement membrane.

PTEN Mutations and the PI3K/AKT/mTOR Pathway

  • Role of the PTEN gene (10q23.31): PTEN encodes a lipid phosphatase that dephosphorylates PIP3 to PIP2, acting as the main brake on the PI3K/AKT/mTOR cell survival pathway.
  • Molecular mechanism in Cowden syndrome: Loss-of-function mutations in PTEN remove the check on PI3K, leading to overactivation of AKT and mTOR. This drives uncontrolled proliferation of outer root sheath cells and stimulates the synthesis and build-up of large amounts of glycogen in the cytoplasm, which gives the cells their characteristic clear appearance.

Desmoplastic Trichilemmoma

  • Features: A benign variant whose microscopic appearance can easily be mistaken for sclerosing (infiltrative) basal cell carcinoma or squamous cell carcinoma.
  • Mechanism: The center of the tumor lobule undergoes marked stromal fibrosis (desmoplastic stroma), which compresses the clear cells into thin, irregular cords and nests. The cells nevertheless remain benign, with few mitoses, CD34 positivity and a preserved hyaline basement membrane.

Histopathologic Criteria

Microscopic Features (H&E)

The lesion sits in the superficial dermis and connects directly to the epidermis or the follicular infundibulum:

  • Well-circumscribed lobules: One or more lobules of cells bulge down into the superficial dermis while remaining anchored to the epidermis.
  • Clear cells: Enlarged cells with abundant pale cytoplasm full of glycogen (PAS-positive and removed by diastase digestion, PAS-D negative).
  • Peripheral palisading and hyaline membrane: The outermost layer of basaloid cells lines up perpendicular to the edge of the lobule (palisading) and rests on a markedly thickened, eosinophilic hyaline basement membrane, similar to the vitreous membrane of the hair follicle.

Imaging and Dermoscopy

  • Dermoscopy:
    • Verrucous hyperkeratotic pattern: A rough surface with alternating opaque bands of keratin.
    • White halo structures: Surrounding plugged follicular openings.
    • Radial or punctate vessels: Evenly distributed at the edge of the lesion, with no abnormal arborizing vessels.
  • High-frequency skin ultrasound: A superficial, well-defined hyperechoic lesion in the epidermis and superficial dermis that does not extend into the reticular dermis.

Clinical Appearance on the Skin

  • Features: Single or multiple small papules (1–5 mm), skin-colored, pale pink or ivory-gray, with a smooth or slightly rough, wart-like (verrucoid) surface.
  • Distribution: Overwhelmingly on the face (especially around the nose, upper lip, cheeks and forehead) and the ears.
  • Clinical significance: The presence of three or more facial trichilemmomas is a core clinical criterion that raises suspicion of Cowden syndrome (PTEN hamartoma tumor syndrome, PHTS).

Multidisciplinary Management and Cosmetic Treatment at Haute-Derma

Patients diagnosed with trichilemmoma at Haute-Derma are managed by a multidisciplinary team: histopathologic/IHC confirmation, PTEN genetic screening and tissue-sparing cosmetic laser treatment.

Histopathology and Genetic Testing

  • Small punch biopsy (2 mm punch): The full thickness of a facial papule is sampled for H&E and an IHC panel.
  • Assessing PTEN expression:
    • If PTEN (IHC) is completely absent in the tumor cells, the PTEN pathway is confirmed to be disrupted, and next-generation sequencing (NGS) is ordered to look for a germline PTEN mutation.

Multi-Organ Cancer Screening (for Patients With Cowden Syndrome)

  • Thyroid: Annual thyroid ultrasound from age 18, with fine-needle aspiration (FNA) of suspicious nodules rated TIRADS 4 or higher.
  • Breast (women): Monthly breast self-examination, plus annual mammography combined with breast MRI from age 30.
  • Uterus and kidneys: Transvaginal ultrasound to screen for endometrial cancer, and kidney ultrasound or MRI every 1–2 years.

Cosmetic Laser Ablation

Because trichilemmoma is a benign lesion confined to the epidermis and superficial dermis, laser ablation is the treatment of choice:

Er:YAG 2940 nm Laser or Fractional CO2 Laser

  • How it works: The 2940 nm Er:YAG wavelength removes the lesion precisely, one microscopic layer at a time, until the papule is level with the surrounding healthy skin.
  • Advantages: Its water absorption is 10 times higher than that of the CO2 laser, so heat spread to surrounding tissue is minimal, the wound crusts over quickly, and no atrophic or hypertrophic scar is left behind.
