Hidrocystoma

Hidrocystoma is a benign cystic proliferation arising from the duct or secretory portion of either the eccrine or the apocrine sweat glands. Although completely benign, hidrocystomas tend to appear in cosmetically important areas of the face (especially around the eye sockets, the eyelids and the cheeks), slightly distort the skin contour, and are easily mistaken for cystic malignancies such as cystic BCC or adnexal carcinoma.

Based on how they form and how they look under the microscope, hidrocystomas fall into two main types:

Eccrine vs. Apocrine: How Each Type Develops

  • Eccrine hidrocystoma (obstructive retention cyst):
    • Mechanism: mechanical or functional blockage of the intraepidermal sweat duct (acrosyringium) or the dermal duct, triggered by heat, humidity and excessive sweating (hyperhidrosis). Trapped sweat stretches the duct below the blockage into a cyst cavity.
    • Clinical variants: the multiple Robinson type, with many small cystic papules on the faces of middle-aged women that swell in hot weather and flatten in the cold; and the solitary Smith type, a single fixed cyst unaffected by the weather.
  • Apocrine hidrocystoma (proliferative type, apocrine cystadenoma):
    • Mechanism: not simply the result of a blockage, but a true epithelial cystic tumor (true cystic adenoma) arising from the secretory coil of an apocrine gland. The lining cells proliferate on their own and continuously secrete protein- and lipid-rich fluid into the cavity.

Genetics and Associated Syndromes

  • WNT10A mutations (Schöpf-Schulze-Passarge syndrome): homozygous or compound heterozygous mutations in WNT10A disrupt Wnt/beta-catenin signaling during development of the skin appendages. Patients have multiple apocrine hidrocystomas of the eyelids, together with palmoplantar keratoderma, sparse hair and missing teeth (hypodontia).
  • PORCN mutations (Goltz syndrome / focal dermal hypoplasia): apocrine hidrocystomas may occur alongside streaks of thinned skin and bone abnormalities.

Our goals in treating hidrocystoma at Haute-Derma are:

  • Accurate differential diagnosis: ruling out cystic carcinomas (cystic BCC, malignant adnexal tumors) with dermoscopy and 50 MHz high-frequency skin ultrasound.
  • Minimally invasive destruction of the cyst wall: a 2940 nm Er:YAG micro-spot laser or insulated RF microneedles destroy the secreting lining of the cyst without leaving depressed scars or loss of pigment around the eyes.

Histopathology and Immunohistochemistry (IHC) Criteria

Characteristic Features Under the Microscope

Eccrine and apocrine hidrocystomas are told apart under the microscope by the cells lining the cyst wall:

  • Eccrine hidrocystoma:
    • Cyst wall: one or two layers of flattened or low cuboidal epithelium.
    • Cyst contents: empty or containing a little pale proteinaceous fluid.
    • Key features: no decapitation secretion (no decapitation secretion) and no distinct myoepithelial layer around the cyst.
  • Apocrine hidrocystoma:
    • Cyst wall: a clear two-layer structure: an outer layer of flattened myoepithelial cells (SMA+, p63+) and an inner layer of tall columnar cells or cuboidal cells.
    • Decapitation secretion: the apical cytoplasm of the lining cells bulges into the lumen and pinches off droplets of mucin or lipid, a hallmark of apocrine differentiation.
    • Cytoplasm: deeply eosinophilic granular cytoplasm containing diastase-resistant, PAS-positive granules (PAS+).

High-Frequency Skin Ultrasound and Dermoscopy Criteria

  • High-frequency skin ultrasound:
    • Shape and structure: a round or oval, well-defined cyst with a very thin wall in the superficial to mid dermis.
    • Echo pattern: a completely anechoic lumen in eccrine cysts, or a hypoechoic lumen with internal debris in apocrine cysts.
    • Posterior acoustic enhancement: the typical ultrasound sign confirming that the lesion contains fluid.
    • Color Doppler: no blood flow signal inside the cyst or in its wall.
  • Dermoscopy:
    • Background: a dome-shaped cyst with a thin, taut, shiny surface.
    • Color (Tyndall effect): a homogeneous bluish-gray, bluish-purple or translucent yellowish tone. The blue color comes from scattering of short-wavelength light as it passes through dark cyst fluid deep in the dermis (the Tyndall effect).
    • Vessels: fine arborizing vessels running around the base of the cyst without entering its center.

Clinical Presentation

  • Common sites: the periorbital area, lower eyelid, inner corner of the eye, cheeks and nasolabial folds.
  • Appearance: single or multiple cystic papules, 1–6 mm (eccrine) or 3–15 mm (apocrine), soft, taut, smooth and shiny.
  • Symptoms and link to weather: NO pain and NO itch. People with the Robinson (eccrine) type often notice the cysts swelling and becoming more obvious in heat, during exercise or after spicy food, then shrinking or flattening in cold weather.

Our Multimodal Treatment Plan at Haute-Derma

Even though hidrocystomas are benign, treating them on the face takes skill: the cyst wall must be fully destroyed to prevent recurrence while keeping the cosmetic result as good as possible (no depressed scars, no hyperpigmentation):

Imaging and Lesion Triage

  • 50 MHz high-frequency skin ultrasound: measures the diameter, wall depth and fluid volume precisely.
  • Clinical typing: we distinguish solitary or multiple eccrine lesions from apocrine hidrocystoma to choose the right procedure.

Minimally Invasive Ablation of the Cyst Wall

2940 nm Er:YAG Laser Roof Vaporization and Cavity Ablation:

  • Our standard technique for lesions on and around the eyelids.
  • A 2940 nm Er:YAG laser (300 µs pulse, 1–1.5 mm spot) opens a tiny hole in the roof of the cyst so all the trapped fluid can drain.
  • An insulated radiofrequency microneedle is then placed directly into the cavity to deliver heat that coagulates and destroys the entire secreting lining (cyst wall coagulation).

Complete Cyst Removal (Surgical Excision):

  • Used for large (> 8–10 mm) solitary apocrine hidrocystomas. The surgeon carefully dissects out the entire cyst capsule intact without rupturing it, then closes the wound with fine 7-0 monofilament sutures.

Maintenance Treatment and Cosmetic Recovery

  • Topical sweat-reducing agents (topical anticholinergics): for patients with multiple Robinson-type eccrine cysts, 0.5–1% glycopyrrolate or 1% atropine solution applied once or twice daily helps prevent recurrence in summer.
  • Exosomes / PDRN: immediately after laser/RF ablation, purified stem cell-derived exosomes combined with PDRN are delivered into the treated area to rebuild the extracellular matrix and prevent depressed scars or post-inflammatory hyperpigmentation (PIH).
  • Low-level light therapy (LLLT, 633 nm): reduces swelling and redness and speeds healing of the tiny wounds within 24–48 hours.
