Cosmeceuticals

Cosmeceuticals sit on the border between cosmetics and pharmaceuticals. The term was popularized by Professor Albert Kligman to describe topical ingredients that act directly on the skin's physiology, cellular structure and biological function while remaining safe enough for everyday use.

In modern aesthetic dermatology, cosmeceuticals do more than keep skin healthy. They are an essential partner to procedures such as laser, chemical peels and microneedling, improving results and shortening recovery time.

Skin Barrier Penetration

The skin barrier is a biological membrane in the stratum corneum often described as "bricks and mortar": dead corneocytes are the bricks, and the extracellular lipids (ceramides, cholesterol and free fatty acids) are the mortar. For a cosmeceutical ingredient to reach the dermis and have a biological effect, it must get past strict physical and chemical barriers.

The 500 Dalton Rule

Molecules heavier than 500 Da (daltons) have great difficulty crossing an intact stratum corneum on their own. For example, high-molecular-weight hyaluronic acid (1,000–1,500 kDa) stays on the surface to hold in moisture, whereas only micro-molecular hyaluronic acid (< 50 kDa) can penetrate deeper.

Routes Through the Stratum Corneum

  • Intercellular pathway: Fat-soluble ingredients slip through the lipid layers between corneocytes. This is the main route for retinoids, vitamin E and BHA.
  • Transcellular pathway: Water-soluble ingredients pass straight through the corneocytes and the lipid layers. This route offers very high resistance.
  • Transappendageal pathway: Ingredients enter through sebaceous glands, hair follicles and sweat glands. Although these make up only 0.1% of the skin surface, they offer a fast entry route for large molecules.

Delivery Systems

  • Liposomes and nanoemulsions: Enclose the active ingredient in a phospholipid bilayer, improving stability, reducing irritation and carrying the ingredient deeper into the skin.
  • Encapsulation: Protects easily oxidized ingredients such as pure vitamin C (L-ascorbic acid) and retinol from light and air.

Five Key Groups of Cosmeceutical Ingredients

Vitamin A Derivatives (Retinoids)

  • Ingredients: retinol, retinaldehyde, tretinoin, adapalene, bakuchiol.
  • Mechanism: bind RAR/RXR receptors in the keratinocyte nucleus, stimulating cell proliferation and speeding up cell turnover, while inhibiting matrix metalloproteinases (MMPs) to protect existing collagen and boost production of type I and III collagen.
  • Best for: skin rejuvenation, acne treatment and fading pigmentation.

Antioxidants

  • Ingredients: vitamin C (L-ascorbic acid, THD ascorbate), vitamin E (tocopherol), ferulic acid, resveratrol, niacinamide (vitamin B3), coenzyme Q10.
  • Mechanism: neutralize reactive oxygen species (ROS) generated by UV light and pollution, preventing oxidative stress and the breakdown of elastic fibers.
  • The classic combination: 15% L-ascorbic acid + 1% vitamin E + 0.5% ferulic acid work synergistically, increasing the skin's protection against sunlight eightfold.

Pigment-Correcting and Brightening Agents (Depigmenting Agents)

  • Ingredients: hydroquinone, arbutin, tranexamic acid, kojic acid, azelaic acid, vitamin C.
  • Mechanism: inhibit tyrosinase (blocking melanin synthesis), block the transfer of melanosomes from melanocytes to keratinocytes (niacinamide), or inhibit the plasminogen/thrombin pathway (tranexamic acid).

Hydroxy Acids (Exfoliants)

  • Alpha hydroxy acids (AHAs: glycolic, lactic, mandelic acid): water-soluble; they loosen the desmosomes between corneocytes, shedding superficial dead cells and helping the skin hold moisture.
  • Beta hydroxy acid (BHA: salicylic acid): oil-soluble; it penetrates deep into pores to clear sebum and reduce inflammation.
  • Polyhydroxy acids (PHAs: gluconolactone, lactobionic acid): larger molecules that exfoliate gently and strengthen the skin barrier.

Peptides and Growth Factors

  • Signal peptides (palmitoyl pentapeptide-4): Signal fibroblasts to produce collagen and elastin.
  • Neurotransmitter-inhibiting peptides (acetyl hexapeptide-8 / Argireline): Mimic the effect of botulinum toxin, relaxing superficial muscles and softening expression lines.
  • Copper tripeptide-1: Promotes wound healing, reduces inflammation and remodels the connective tissue matrix.

Managing Side Effects and Irritation From Cosmeceuticals

Because cosmeceutical ingredients are biologically potent, they always carry a risk of side effects if the regimen is not tailored to the individual:

  • Retinoid reaction (retinization): Redness, peeling, dryness and stinging during the first 2–4 weeks of retinoid use. It can be managed with the "sandwich technique" (moisturizer applied before and after the retinoid) or by using the product less often.
  • Post-inflammatory hyperpigmentation (PIH): Occurs when hydroxy acids or high-strength vitamin C are applied to damaged or irritated skin.
  • Irritant or allergic contact dermatitis: Usually caused by excipients, preservatives (parabens, phenoxyethanol) or added fragrance.
