Periorbital Aging

Periorbital aging is among the earliest and most complex signs of facial aging. The skin around the eyes is the thinnest on the body (only about 0.5 mm), has very few oil and sweat glands, and is put through more than 10,000 blinks and squints every day.

Over time, aging around the eyes goes well beyond crow's feet. It reflects a loss of support at several levels at once: shrinking of the deep orbital fat, widening and resorption of the bony orbit, bulging of the lower-lid fat pads (orbital fat herniation), upper-lid hooding and brow ptosis, and marked breakdown of the collagen network in the dermis.

At Haute-Derma, we assess the eye area and plan rejuvenation on sound medical principles, preserving your natural anatomy as much as possible and putting the safety of your eyes first.

How the Eye Area Ages: Biology and Anatomy

Aging around the eyes happens in several anatomical layers at the same time:

  • Bone changes: The bony rim of the orbit gradually widens and loses volume. The soft tissues above lose their natural platform, and hollows form around the eye.
  • Fat bulging and deep fat loss: As the orbital septum weakens, the front fat compartments push forward and form lower-lid bags. Meanwhile the deep fat compartments shrink, so the contrast between hollow and bulge becomes sharper.
  • Changes in the orbicularis oculi muscle: Years of repeated contraction etch radiating lines at the outer corners (crow's feet). As the muscle loses tone, the upper lid also starts to droop.
  • Declining skin quality: Falling levels of collagen and hyaluronic acid thin the eyelid skin, making blood vessels show through and fine crepey lines appear.

Grading Periorbital Aging

GradeClinical signsAnatomical changes
Grade 1 (Mild)Crow's feet when smiling, thin eyelid skin, early slight hollowing of the tear trough.Collagen breakdown in the superficial dermis; mild orbicularis contraction.
Grade 2 (Moderate)Crow's feet visible at rest, deep tear trough, early lower-lid bags.Mild laxity of the orbital septum; loss of deep orbital fat.
Grade 3 (Severe)Obvious lower-lid bags, marked upper-lid droop, deeply hollow eye sockets.Clear orbital fat herniation, resorption of the orbital rim, significant excess skin.

In-Depth Assessment at Haute-Derma

Before any treatment, a Haute-Derma dermatologist carries out a thorough examination to pin down the cause and degree of aging:

  • Classifying dark circles: We distinguish structural circles (shadowing from the tear trough), vascular circles (thin skin revealing veins) and pigmentary circles caused by melanin.
  • Assessing elasticity: We check how lax the orbital septum is and how far the fat has herniated, which guides the choice between non-surgical and surgical options.
  • Studying muscle movement: We evaluate how the orbicularis oculi and brow muscles move so that any adjustment stays balanced and harmonious.

Treatment and Eye-Area Rejuvenation

Depending on each patient's findings, our doctors combine treatments across several layers for a natural, long-lasting result:

  • Deep layer (structure and volume): Highly biocompatible fillers designed for this area are placed with a dedicated needle close to the bone in the tear trough, restoring support and softening the shadow cast by hollowing.
  • Dynamic muscle layer: Micro-dose injections at the outer eye and brow relax the overactive muscle fibers that cause wrinkles, while gently opening and lifting the outer corner of the eye in a natural way.
  • Surface and dermal layer: Micro-injections of skin-nourishing ingredients are combined with safe fractional energy-based devices to stimulate collagen, thicken the eyelid skin and brighten the area around the eyes.
