Microneedling Radiofrequency

Microneedling radiofrequency (MRF) is an advanced minimally invasive aesthetic technology that works through two mechanisms at once: mechanical micro-injury from fine needles and accumulated thermal injury from radiofrequency energy delivered directly into the dermis. Unlike traditional light-based resurfacing, MRF carries heat through microsurgical needles that pass through the epidermis without damaging the skin surface, which removes the risk of burns or loss of pigment.

At Haute-Derma, MRF is a cornerstone of our protocols for complex atrophic scars, large pores, stretch marks and loss of elasticity in aging skin. Precise control of needle depth in 0.1 mm steps, together with the choice of insulated or non-insulated needles, lets us tailor the clinical result to each individual's skin.

Biological Mechanism and Thermal Layering in the Dermis

Combining mechanical needle penetration with RF heat remodels the skin on several levels:

  • Mechanical micro-injury: Sterile microneedles enter the skin at high speed, creating thousands of tiny micro-channels. This triggers the natural wound-healing response and releases platelet-derived growth factors (PDGF, TGF-beta) that set cell renewal in motion.
  • RF thermal coagulation zone: As soon as the needle tips reach the target depth in the dermis (0.5–5.0 mm), RF energy is released, raising local tissue temperature to 60–70°C. This heat creates micro-thermal coagulation zones (MCZs) that denature aged collagen fibers and stimulate fibroblasts to produce new type I and III collagen and elastin.
  • Sebaceous gland suppression and scar remodeling: Heat from the needle tips can selectively shrink and destroy enlarged sebaceous glands, giving a lasting reduction in oiliness and helping to control acne. The same thermal energy breaks the fibrous bands that tether scars down, allowing new connective tissue to fill in the depressions.

Comparing MRF Needle Types

FeatureInsulated NeedlesNon-Insulated Needles
Needle DesignShaft coated with insulation; only the tip (0.3–0.5 mm) conducts currentThe entire needle conducts current from base to tip
Target Heating ZoneConcentrated only in the deep dermis, where the needle tip stopsSpread along the full length of the needle, from epidermis to dermis
Clinical AdvantagesProtects the epidermis, with almost no risk of PIH; safe for darker skin (Fitzpatrick IV–V)Remodels every tissue layer; highly effective for surface rejuvenation, pore reduction and fine wrinkles
Recovery Time1–2 days (mild pinkness, no peeling)3–5 days (fine pinpoint crusting)
Preferred Indications at Haute-DermaDeep atrophic scars (boxcar/icepick), double chin, sagging, sensitive skin prone to pigmentationLarge pores, stretch marks, shallow fibrotic scars, rough uneven texture

Clinical Procedure and Treatment Protocol at Haute-Derma

  • Skin Assessment and Pre-Treatment Preparation The doctor assesses skin thickness in each area (forehead, cheeks, chin) and classifies the scar type or degree of aging. Medical numbing cream (lidocaine 5%) is applied for 45–60 minutes, and the skin is cleansed with chlorhexidine 0.12% antiseptic solution.
  • Needling Technique and MRF Settings
    • Depth adjustment: The doctor sets needle depth by area: 0.8–1.2 mm for the forehead and around the eyes; 1.5–2.5 mm for the cheeks and acne scars; 2.5–3.5 mm for the double chin and stretch marks.
    • Handling technique: The handpiece is held at 90° to the skin surface so the needles enter cleanly without shearing. Insertion force and pulse duration are calculated according to the thickness of the target tissue.
  • Enhanced Recovery Through Micro-Channels Taking advantage of the thousands of micro-channels just created, the doctor applies concentrated biological products such as exosomes, PRP or low-molecular-weight HA so they penetrate deep into the dermis, doubling the speed of tissue recovery and calming the skin straight away.

Managing Complications and Clinical Safety

  • Post-inflammatory hyperpigmentation (PIH): Can occur when non-insulated needles are used at excessive energy on darker skin. It is managed with a lipid-barrier repair cream, broad-spectrum mineral sunscreen and, once the skin has settled, prescription tranexamic acid or hydroquinone 2–4%.
  • Acneiform eruption: Caused by heat stimulating the sebaceous glands or by poor aftercare hygiene. It is treated with a topical antibiotic (clindamycin) and cleansing with normal saline.
  • Grid pattern marks: These appear when the handpiece is pressed unevenly or energy is fired before the needles are fully inserted. A hydrating serum containing panthenol (B5), hyaluronic acid and epidermal growth factor (EGF) helps the grid-shaped crusts fade within 5–7 days.
