Soft-Tissue Augmentation

Soft-tissue augmentation is a group of non-surgical cosmetic procedures that restore volume lost with aging, refine facial contours, and soften deep static wrinkles. By placing biocompatible dermal fillers or the patient's own fat into the appropriate anatomical layers, it compensates for the loss of superficial and deep fat and for natural age-related bone resorption.

At Haute-Derma, soft-tissue augmentation is guided by one principle: anatomical safety comes first. Each treatment carefully matches the right filler to layered injection techniques (micro-droplet, bolus, fern) and the use of blunt micro-cannulas, to create natural, balanced contours while keeping the risk of vascular occlusion as low as possible.

The Anatomy of Facial Aging and Layer-Targeted Injection

Facial aging happens on several levels at once, from bone and ligaments to fat compartments and skin:

  • Bone loss and shrinking deep fat: The eye sockets widen, the upper jaw recedes, and the deep fat compartments (deep medial cheek fat, deep pyriform space) shrink. These areas call for supraperiosteal injection of a firm, high G-prime filler to rebuild support and lift the overlying tissue.
  • Descent of superficial fat and static lines: As the retaining ligaments loosen, the superficial fat pads slide downward, creating nasolabial folds, marionette lines, and hollow temples. Here, a soft, flexible filler is placed in the subdermal or superficial fat layer to smooth the surface without lumps or bulging.

Types of Soft-Tissue Fillers

Filler TypeBiocompatibility and DurationPreferred Uses at Haute-Derma
Hyaluronic acid (HA)Very high / 6–18 months (depending on cross-linking)Nasolabial folds, lips, chin, temples, under-eye hollows
Calcium hydroxylapatite (CaHA)High / 12–24 months (stimulates collagen)Jawline contouring, back-of-hand rejuvenation, chin
Poly-L-lactic acid (PLLA)Good / 18–24 months (stimulates type I collagen)Major volume loss across the face, hollow cheeks, temple hollows
Autologous fatComplete / long-lasting (once the graft survives)Full-face rejuvenation, temples, tear troughs

Our Clinical Process and Safe Injection Techniques

  • Anatomical Assessment and Mapping of Danger Zones: The doctor locates the course of the major vessels (such as the facial, angular, superficial temporal, and supraorbital arteries) to create a safe injection map.
  • Layered Injection Techniques
    • Supraperiosteal bolus technique: used where structural support is needed (chin, cheekbones, forehead). Injection is slow and gentle, always with 5–10 seconds of aspiration before the filler is delivered.
    • Micro-cannula technique (22G–25G): blunt-tipped cannulas are used in areas with complex vascular networks (nasolabial folds, temples, under-eye hollows), pushing vessels aside instead of piercing them.
  • Immediate Post-Injection Assessment and Molding: The doctor gently molds the filler so it spreads evenly along the natural anatomy, avoiding lumps or misplacement between layers.

Managing Complications and Preventing Vascular Occlusion

  • Vascular occlusion / ophthalmic artery occlusion (blindness): The most serious complication, occurring when filler is injected directly into a vessel, cutting off blood supply to tissue or traveling backward into the ophthalmic artery.
    • Warning signs: sudden severe pain during injection, skin blanching, dusky purple discoloration along the course of a vessel, or reduced vision.
    • Haute-Derma emergency protocol: stop injecting immediately. Inject high-dose hyaluronidase (1500 IU) throughout the entire ischemic area; apply warm compresses, massage firmly, apply nitroglycerin paste, and give aspirin/anticoagulants.
  • Delayed-onset granuloma: a foreign-body immune reaction to the filler. Treated with intralesional corticosteroids, hyaluronidase, or a combination with antibiotics.
