Lichen Nitidus

Lichen nitidus is a rare, chronic inflammatory skin condition marked by numerous tiny papules (1–2 mm) that are flat-topped, shiny and skin-colored or pale pink. Its cause is not fully understood, but under the microscope it is closely related to lichen planus. Its defining feature is a cellular infiltrate confined to a single dermal papilla and cupped by the surrounding epidermis, which gives the lesions their strikingly uniform appearance.

At Haute-Derma, our approach to lichen nitidus puts safety first, avoids unnecessary invasive procedures and focuses on helping the lesions clear on their own more quickly.

Diagnosing Lichen Nitidus

At Haute-Derma, diagnosis relies on close analysis of fine details to tell lichen nitidus apart from keratotic conditions and sweat gland tumors:

  • Assessing the papules: We look for uniformly sized papules with flat, glossy, almost mirror-like tops, clustered on the genitals, the front of the forearms, the backs of the hands or the trunk.
  • Dermoscopy: A key tool that shows whitish, structureless, cloudy areas and loss of the normal pigment network over each papule.
  • Looking for the Koebner phenomenon: We check for papules lined up along scratches or areas of minor skin injury.
  • Skin biopsy: Shows the classic "claw clutching a ball" pattern, in which elongated rete ridges wrap around a focal infiltrate of lymphocytes and epithelioid histiocytes within a single dermal papilla.
  • Differential diagnosis: We rule out conditions such as keratosis pilaris, small-papule lichen planus and flat warts.

Managing and Treating Lichen Nitidus at Haute-Derma

Because lichen nitidus is usually not itchy and can clear on its own within months to years, our treatment plans are flexible and guided by each patient's cosmetic goals:

  • Phototherapy (NB-UVB): Narrowband ultraviolet B is regarded as the most effective way to flatten widespread papules, without pain or whole-body side effects.
  • Mid-potency topical corticosteroids: Anti-inflammatory creams quickly reduce how raised the papules are and calm the local immune reaction during active phases.
  • Topical calcineurin inhibitors (TCIs): An excellent choice for sensitive areas such as the face or genitals, controlling lesions long term without the risk of skin thinning.
  • Topical retinoids: Vitamin A derivatives help normalize skin cell differentiation, leaving the surface smoother and more even in tone.
  • Reassurance and follow-up: We explain the benign nature of the condition to ease anxiety and monitor the lesions regularly as they fade on their own.
