Nd:YAG Laser
The Nd:YAG (neodymium-doped yttrium aluminum garnet) laser is one of the foundational and most versatile laser systems in modern cosmetic dermatology. Working on the principle of selective photothermolysis, it uses a neodymium-doped garnet crystal to produce a beam that targets specific chromophores, such as melanin, hemoglobin or tattoo ink particles, while limiting damage to the healthy tissue around them.
Its biggest advantage is that it offers two wavelengths (1064 nm and 532 nm) and flexible pulse modes, from nanosecond (Q-switched) to millisecond (long-pulsed). That flexibility lets a dermatologist tackle a wide range of complex skin problems, from deep pigment and vascular lesions to hair removal and surface rejuvenation.
At Haute-Derma, Nd:YAG laser treatment is built into standardized medical protocols with precise energy control, aiming for the best results and a high level of safety for each skin type.
Selective Photothermolysis and Nd:YAG Pulse Modes
How well Nd:YAG works depends on matching the wavelength and pulse duration to each type of lesion:
- 1064 nm (low absorption by surface melanin, deep penetration):
- This long wavelength reaches deep into the dermis and subcutis (3–5 mm). Because epidermal melanin absorbs it poorly, it is a safer choice for darker skin types (Fitzpatrick III–VI) and helps reduce the risk of post-inflammatory hyperpigmentation (PIH).
- Ideal for deep pigment (dermal melasma, nevus of Ota), deeper vessels, hair removal and collagen remodeling.
- 532 nm (strong affinity for superficial melanin and hemoglobin):
- Produced by passing the beam through a frequency-doubling (KTP) crystal, this wavelength is strongly absorbed by epidermal melanin and by oxyhemoglobin in superficial capillaries.
- Suited to superficial pigment (freckles, sun spots, age spots), fine telangiectasias and cherry angiomas.
- Q-switched (nanosecond) vs. long-pulsed (millisecond):
- Q-switched mode: delivers very high energy in an extremely short burst (nanoseconds). The photomechanical effect shatters pigment into tiny fragments for macrophages to clear; it is also used for tattoo removal and carbon laser rejuvenation.
- Long-pulsed mode: delivers energy over a longer pulse (milliseconds). The photothermal effect coagulates blood vessels or destroys hair follicles with heat.
Clinical Uses of Nd:YAG by Wavelength and Mode
| Wavelength and mode | Main target (chromophore) | Conditions / clinical indications |
|---|---|---|
| 532 nm (Q-switched / Nd:KTP) | Epidermal melanin and superficial hemoglobin | Freckles, age spots, brown spots, telangiectasias on the eyelids or sides of the nose |
| 1064 nm (Q-switched / toning) | Dermal melanin and free pigment | Dermal melasma, nevus of Ota, black or blue tattoo removal |
| 1064 nm (long-pulsed) | Vascular hemoglobin and follicular melanin | Leg telangiectasias, small reticular veins, medical hair removal |
| 1064 nm (sub-millisecond / Genesis) | Dermal microvessels and fibroblasts | Overall skin rejuvenation, smaller-looking pores, less redness |
Pre-Treatment Assessment at Haute-Derma
Before any Nd:YAG treatment, our dermatologists at Haute-Derma carry out a careful clinical assessment:
- Fitzpatrick skin typing and multispectral skin imaging: we determine how deep the pigment lies (epidermal or dermal) so we can choose the right wavelength (532 nm or 1064 nm) and fluence.
- Test spot: for sensitive skin or complex melasma, we fire a few test pulses on a hidden area and watch the response to make sure there is no overheating or loss of pigment.
- Ruling out contraindications: before the procedure we check for photosensitizing medications, high-strength retinoids, or acute dermatitis in the treatment area.
Multimodal Treatment Protocols at Haute-Derma
At Haute-Derma, Nd:YAG is rarely used on its own; it is built into multimodal treatment plans:
- Pigment and mixed melasma (laser toning + mesotherapy): low-energy, high-frequency 1064 nm toning gently breaks up deep melanin, combined with microinjections of pigment-inhibiting nutrients (PDRN with brightening actives) to rebuild the skin barrier from within.
- Surface rejuvenation and pore refinement (Hollywood Carbon Peel): a medical-grade carbon mask is applied to the skin, then the 1064 nm laser makes the carbon particles burst. This deep-cleans the sebaceous ducts, removes old surface cells and stimulates collagen production in the dermis.
- Superficial vessels and telangiectasias: long-pulsed 1064 nm or 532 nm, with carefully chosen spot size and pulse duration, collapses and seals dilated vessels while limiting heat damage to the surrounding tissue.
